In this picture we present a sex reassignment, from woman to man. In this cases we perform a microsurgical reconstruction of the penis using skin from the forearm. Microvascular anastomosis are performed to vessels of the lower limb, and sensitive nerves from the clitoris are sutured to the nerves of the flap.
The most important fact in these cases is the risk of orine fistula. We perform a coverage of urinary ways with a muscle flap from the leg, minimising the risk of fistula.
At the same time, mastectomy, histerectomy and ooforectomy can be performed, two teams working at the same time. Time of surgery does not last more that seven hours.
After some months, when the penis is sensate, we introduce a free fibula flap, so erection can be performed.
